At Little Dolphins Swim Academy, we strive to keep open lines of communication with our teachers and staff. It’s sometimes tough to find time to talk with a teacher before or after your students class, or update the necessary parties regarding a schedule change. Our forms below are designed to help keep the flow of communication open and help with scheduling changes that may be necessary on occasion. Simply review, complete and submit this form back to us at LDSA or by emailing it to info@swimldsa.com.

Parent/Teacher Communication Form

For questions and /or comments regarding your class.

Vacation Suspension Form

You may suspend your membership and hold your place in class for $5 per week, for up to four weeks each year. This request must be submitted a minimum of 30 days in advance of the dates requested.

Cancellation Form

The cancellation request allows you to cancel your lessons and removes you from our swim schedule therefore stopping the monthly automatic deductions. This request must submitted no later than the 25th of the month prior to the first of your last month. (This allows our staff time to adjust the billing.)

Did you know? May 15th is National Water Safety Day. Drowning is the second leading cause of child deaths between the ages of 1-14. Make sure that the perimeter of your pools is fenced off and that you are always watching young swimmers to ensure they are safe in the pool, in the ocean, or any body of water.
