At Little Dolphins Swim Academy, we strive to keep open lines of communication with our teachers and staff. It’s sometimes tough to find time to talk with a teacher before or after your students class, or update the necessary parties regarding a schedule change. Our forms below are designed to help keep the flow of communication open and help with scheduling changes that may be necessary on occasion. You may suspend your membership and hold your place in class for $5 per week, for up to four weeks each year. This request must be submitted a minimum of 30 days in advance of the dates requested.
The cancellation request allows you to cancel your lessons and removes you from our swim schedule therefore stopping the monthly automatic deductions. This request must submitted no later than the 25th of the month prior to the first of your last month. (This allows our staff time to adjust the billing.)
