Parent Teacher/Communication Form

For questions and /or comments regarding your class.

Vacation Suspension Request Form

You may suspend your membership and hold your place in class for $5 per week, for up to four weeks each year. This request must be submitted a minimum of 30 days in advance of the dates requested.

Cancellation Form

The Cancellation request allows you to cancel your lessons and removes you form our swim schedule therefore stopping the monthly automatic deductions. This request must submitted no later than the 25th of the month prior to the first of your last month. (This allows our staff time to adjust the billing.)

Class Make-up/Drop In Form

Each enrolled student is allowed up to one Make-Up or Drop In each month. There is a $10 fee (waived with a doctors sick note for makeups). Makeups are not accumulated from month to month.
